Dr. Mahtab Moradi

Mahtab Moradi, LMFT

Behavioral Health Project Manager

Mahtab Moradi has been a licensed clinician with a specialty in Marriage and Family for over 22 years. She
graduated from the University of Texas in Austin with her bachelor’s in psychology and a minor in Education.
She returned to her hometown of Houston to complete a masters in Family Counseling and a post Masters
fellowship in Behavioral Medicine from the University of Texas Medical Branch.

Her career as a counselor began in serving the low income, indigent population in the clinics of greater
Galveston County. In 2003, she took the role of staff clinician at Devereux Advanced Behavioral Health
working with young adults hospitalized for inpatient care with co-morbid diagnosis and schizophrenia. After
11 years with Devereux, Mahtab served on the Devereux executive board as Treasurer and continued to
contribute to the mission of advancing mental health initiatives impacting youth and young adults.
In 2013, Mahtab joined a faith-based group practice as a co-director of clinical services with South Shore
Center for Couples & Families. In this role, she helped raise awareness for mental health services through
community engagement, pro bono training, public speaking, and promoting mental health education in
schools.

In 2019, Mahtab joined Formulamed as Managing Partner and Director of Behavioral Health. In this role, she
has been able to integrate the role of mental health into workforce development, promoting growth mindset
models and preventative health care versus treating “illness”. Formulamed leadership has been able to help
larger corporations and higher education institutions, including law schools both in Texas and California shift
corporate cultures and improve productivity and employee retention.
In 2023, Mahtab took on the role of project manager for a pilot SAHMSA project which was intended to
address the issue of affordability and access for underserved communities in southwest Houston for adults.
In this role, she has helped promote mental health in communities who traditionally face higher stigma and
SES barriers, impacting more than 10,000 patients. By integrating behavioral health services into the family
medicine clinics, more patients have been able to access the help they need.
